Statement of Credentials
Thank you for selecting me to help meet your ministry needs. It is a privilege to serve you, and I will do all I can to help with whatever needs you have and provide the highest quality of service. Please be aware that the services I provide are Biblical counsel, relational skills, and Inner Healing Prayer only. I am not licensed as a professional counselor and do not provide professional counseling services or psychiatry services (medicine prescriptions.)
My goal is to do what I can as a caring and compassionate person to help you achieve more freedom in Jesus Christ through facilitated inercessory prayer, spiritual warfare, biblical truth, and relational/maturity skills. The following information is provided to serve you better. Please examine it carefully. I will be happy to answer any questions regarding items for which you need additional clarification.
Contact Outside of Session
I will make reasonable efforts to be available for additional sessions or emails on an as-needed basis outside of scheduled session time and as time permits, however, this is not always available. The following rules apply to texting, phone calls, and email:
To ensure I maintain the emotional capacity to meet your needs, prayerful consideration will be given to deciding if, when, and how to respond to all forms of communication between sessions.
You may text me at your discretion.
You recognize and understand that I cannot guarantee your confidentiality when texting.
You may not receive an immediate response to your text and I will only respond as time allows.
You agree not to text any threats of harming yourself ot others to me. If you are feeling suicidal or homicidal, you agree to call 911, a pastor, or a friend for assistance.
You may call me or leave a voice message, however, messages left may not be checked immediately.
You may email me, however, messages may not be checked immediately.
Confidentiality
Confidentiality for Prayer Minister: I am dedicated to preserving the confidentiality and privacy of all my participants. However, some state and federal laws require that I disclose information in certain situations. Please review the following conditions in which I must breath confidentiality:
If I suspect a child, elder, or disabled person is being abused or neglected, I am required to report that information to a state agency.
If a participant brings charges against me.
If a court orders my testimony of your records.
I may, at times, talk with another professional about your case in order to obtain an objective point of view. In those instances, your confidentiality will be maintained and no identifying information will be revealed, only the circumstances of your situation. Professional ethics will also require any professional with whom I consult to maintain your confidentiality.
When I believe a client is in danger to themselves or others (suicidal or homicidal).
Confidentiality for Participant: You may discover that someone you know is also a participant in ministry with me. In these cases, the individual's participation should be kept confidential. Please do not ask the individual why they are participating. You may feel free to share why you are participating if you choose, but do not expect them to reciprocate.
Appointments
General: Sessions are scheduled to last 90 minutes. I usually build in 30 minutes of additional time in the session if more time is needed.
Cancelation: If you need to cancel an appointment, please inform me as soon as possible. I am always glad to reschedule.
Right to Refer
Referrals: To better serve you or if you could benefit from a ministry or counseling modality that I cannot provide, I will recommend an alternative option for you. You have the right to ask about such other treatments, their risks, and their benefits.
Limitation of Liability
You acknowledge that Jim Teague, as a Prayer Minister, has voluntarily agreed to pray with you using various inner healing prayer methods. You understand this is a Christian ministry activity and not professional counseling. You understand that Jim Teague is not a licensed counselor. You understand that Jim Teague is doing what he can as a caring and compassionate person to try to help you achieve more freedom in your life. You understand that Jim Teague is not guaranteeing any results from your ministry time together.
You further state that you have voluntarily sought the assistance of your own initiative and are under no obligation to accept or implement any of the advise or help that you might receive from Jim Teague. You understand that you are free to reject any or all of the assistance or recommendations you may receive through this ministry activity.
You agree to hold Jim Teague harmless and free from any and all liability, loss, or damage, real or perceived, that may arise from your interactions. In the event of any claim for personal injury, which includes mental, emotional, and spiritual health, you, your heirs, executors, administrators, or assigns agree to indemnify and hold harmless Jim Teague from any and all claims or causes of his actions or by any other person or entity by whomever or whatever made or presents. Under no curcumstances will you present any claim against Jim Teague for personal injuries or otherwise, caused by any act or perceived act of negligence by Jim Teague.
By signing below, I attest that I have read this consent in its entirety. I understand any potential risks of engaging in this ministry model with Jim Teague and assume all risks associated with this ministry. I agree and understand this to be a complete and unconditional release of all liability to the greatest extent allowed by law. If any portion of the release is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect. I understand that signing this document may affect my legal rights.
